When coming for an appointment at Clarity be sure to bring your health insurance card, Medicaid, or Medicare card along with your drivers license.
Upon arrival for your (or your child’s) appointment at Clarity you will receive a copy of Clarity’s Notice of Privacy Practices to review.

The client is ultimately responsible for all costs not covered by their insurance company. These costs include, but are not limited to: services that have been provided which are not covered by the client’s insurance policy, balances after the insurance payment, or failure to obtain prior authorization before your appointment.

Please note that Clarity’s psychologists are not contracted with any insurance providers. Although the client will be given the necessary paperwork following the evaluation to file a claim with their health insurance company, full payment for Psychological Evaluation Services is the responsibility of the client (or parent/guardian) and it is due prior to the first appointment. A deposit of one half of the fee is due two weeks prior to the first appointment and a deposit of $50 is is required to secure an appointment for psychological evaluation services.
We understand that services provided at Clarity may be cost prohibitive. For that reason we have financing and payment options available for our clients.
We will assign a therapy time specifically for your child. If a client or family cancels a therapy session, for any reason, we reserve the right to charge you for the therapy session (example: client sickness, vacation, etc.). We will try to make up client absences that are due to sickness, but unfortunately we cannot guarantee a new therapy time. If our team is unable to conduct the therapy session due to therapist absence, your account will be credited for those sessions. Two consecutive unexplained client absences or 25% of therapy sessions missed may warrant dismissal from speech therapy services. In addition, any account that has a client balance due past 30 days will also result in dismissal.
Therapy sessions begin and end at scheduled times. There are no provisions for extending sessions to accommodate client tardiness. If a client is more than 10 minutes late for a session it is up to the therapist’s discretion as to whether they will see that child.

Please note, we are an out of network provider for psychological evaluation services. However, if you choose to file a claim yourself, please inform our billing office, so we can help make your filing easier by providing a receipt that includes service codes, fees, payment, and diagnostic codes. Insurance companies vary in terms of reimbursement for psychological testing, so please contact your insurance company before beginning the evaluation process, if you are planning to seek reimbursement. Please be aware that third party insurance providers seldom reimburse for academic/learning related issues, although policies vary. Payment is the responsibility of the client or parent/guardian unless prior arrangements are made.
Clarity Cares is an income-based sliding scale fund that allows Clarity to offer some services at a reduced rate to individuals without the ability to pay full fees and/or without apt insurance coverage.
