Out of Network Services
While Clarity is a contracted provider for multiple insurance plans, our psychological evaluation services are not a contracted service with any insurance provider.
Payment is the responsibility of the client or parent/guardian and is due in full prior to the first appointment. A nonrefundable deposit of $50 is is required to secure an appointment for psychological evaluation services and a second deposit payment of one half of the fee is due two weeks prior to the first appointment.
If you are planning to file a claim for reimbursement of fees paid to Clarity for psychological evaluation services, it is recommended that you call your insurance company about any requirements (including preauthorization requirements for services rendered by out of network providers). The specific procedures codes that are used for these services are: 90791, 96136, 96137, 96130, and 96131. Please be aware that most insurance providers seldom reimburse for academic/learning related issues, although policies vary. We will provide you with a superbill listing the procedures, units, fees, fees paid, and diagnoses with the full report when the evaluation process is complete.
We understand that psychological testing services provided at Clarity may be cost prohibitive. For that reason we have financing and payment options available for our clients.
