Payment
Payment is required at the time of service. This policy applies to applicable and estimated deductibles and co-payments under your health insurance policy, provided we are in-network with your plan. If you do not have health insurance or are receiving a service that is not covered under your policy, we require full payment at the time of service.
Pay your bill online in the Patient Portal
We are excited to introduce our Clarity family to our new Patient Portal where you can pay your bill. If you are an existing patient, please register for access to your Patient Portal below to pay your bill.
If you have any questions about a bill you have received from Clarity, understanding an insurance EOB for services provided by Clarity, or if your health insurance plan will pay for your visit to Clarity our team is prepared to help you find an answer to those questions. Appointments
We will make a copy of your driver’s licensed and insurance card(s) so please be sure to bring both to all appointments.
If you are a new patient, please arrive at least 30 minutes prior to your appointment time and please complete the registration forms (click here to download) for your appointment prior to your arrival. Included in your New Patient Form packet is a copy Clarity’s Notice of Privacy Practices for your review. Please let us know if you would like a printed copy and we will be happy to provide one to you. If you have any questions about fees that will be due at the time or service or if your health insurance plan may pay for your visit to Clarity, our team is prepared to help you find an answer to those questions. Cancellations/Makeups
We will assign a therapy time specifically for your child. If a client or family cancels a therapy session, for any reason, we reserve the right to charge you for the therapy session (example: client sickness, vacation, etc.). We will try to make up client absences that are due to sickness, but unfortunately we cannot guarantee a new therapy time. If our team is unable to conduct the therapy session due to therapist absence, your account will be credited for those sessions. Two consecutive unexplained client absences or 25% of therapy sessions missed may warrant dismissal from speech therapy services. In addition, any account that has a client balance due past 30 days will also result in dismissal.
Late Arrival
Therapy sessions begin and end at scheduled times. There are no provisions for extending sessions to accommodate client tardiness. If a client is more than 10 minutes late for a session it is up to the therapist’s discretion as to whether they will see that child.
